Accurate information is the key to getting good health care. Doctors need the right data to give the best treatment.
That's why health officials urge people to create a personal health record...to help you keep track of your health care information. It can be as simple as a paper file or it could be an electronic file kept on your computer. Whichever form you choose, be sure to include the medicines you take, your allergies and your medical history. This information that makes you medically unique and can have a huge impact on your treatment. Be as thorough as possible.
Having this information organized and readily available makes you an active partner in your health care and allows you to ask the best questions to stay safe and healthy.
